Placement Planning Meeting (PPM)
This is an initial meeting between yourself, social worker and the placement staff usually conducted within the firs 3-5 days of your placement. During this meeting is where we discuss important information about your placement such as rules of placement, our expectations, your expectations, curfew, weekly allowance, contact with family etc and anything you may wish to discuss.
Key-Working
Refers to the support sessions that are centred around equipping you with independent skills in all aspects of your life. Key working sessions will explore things like health, education, training or employment(ETE), relationships, Identity, cooking, cleaning, finances and budgeting etc.
Mentoring
Refers to the support sessions that are centred around doing things you enjoy together with staff. This involves things such as watching movies, recording or writing music, going go-karting, bowling, playing basketball, swimming etc whatever your interests are. These activities are aimed towards your enjoyment and helping you build foundational relationships with staff and discover more about yourself.
Key-working Codex
This document outlines the work that will be done with you during your key working sessions such as education, training or employment (ETE), relationships, Identity, cooking, cleaning, finances and budgeting. This way you are aware what each step entails and you are involved in your support.
Independent Skills assessment
This is a document that is used by yourself, staff and social worker to track your progress in various independent skills like cooking, cleaning, budgeting, health, relationships etc. Staff, social worker and yourself will be able to better identify your progress, strengths, and weakness.
Support Plan
This is a document that identifies all your strengths, weaknesses and risks. The plan will set objectives and timeframes on how risks will be reduced, your strengths enhanced, and weaknesses turned into strengths. The plan is centred around you and makes into consideration your views and opinion and how you access support better. You will be involved in the making and updating of you support plan.
