An overview of your
Placement Agreeemnt
We want to keep our placement as a safe and homely environment for all of it's residents. To do that we need to ensure that there are clear rules that allow all people to be comfortable. If you aren't clear on any of the rules, we are happy to discuss them with you in more detail.
The rules listed here are those agreed to in your behaviour and tenancy agreement
I agree to
I, the young person, agree to respect and abide by this agreement in order to join and remain with Infinity Alliance Services.

not bring alcohol or illegal substances onto the premises.

I will show respect towards the staff and other residents and not display any violence or threatening behaviour as I know this is completely unacceptable and will not be tolerated.

I will Always notify a member of staff and sign in and out when leaving the premises using the signing in book.

I will not have more than 1 (one) guest at the houses at any one time, and I will ensure that any guest I have leaves by 8pm.

I will give appropriate notice of at least 24 hours before bringing guests to the house, so that I can be sure that this will not clash with meetings or scheduled visits or activities. I understand that staff have the right to refuse my visitors if they deem it appropriate to do so.

I understand that for the safety of myself, staff and other residents and staff I will not be allowed to have any guests stay overnight.

I agree to pay for any deliberate damage to the house that I cause. I understand that this will be deducted from my weekly allowance, and serious or ongoing damage caused to the house may result in me losing my placement.

I understand that if I am supposed to be in a scheduled activity or school, I will not be permitted to have guests at the house during the times of school or the scheduled activity.

I understand that my guests are my responsibility.

To protect the safety of other residents and staff, I will not leave any of my guests alone in the house at any time.

I will be considerate of neighbours and keep my music to reasonable levels and turn down to quiet levels after 10pm.

I agree to keep to my scheduled. This is a time that has been agreed by my social worker, I understand that this can change if my social worker agrees, and this contract may be updated.

I understand that if I am out past my curfew, I may be reported missing to the police and to my Local Authority’s Out of Hours Emergency Team, This will be noted in my file and may impact my placement.

I understand that as part of my placement I am expected to remain in placement for at least 4 days of the week. Should I stay out of the house, I understand that I need to ensure that placement knows where I will be staying, by providing an address and any contact details of an appropriate adult so that they are able to verify this information.

I understand that if I do not send correct or appropriate details of my whereabouts, I will be reported missing to the police and also to my Local Authority’s Out of Hours Emergency Team.

I understands that if I am out past my curfew or do not return on a day when I am supposed to, but have clearly communicated my whereabouts with staff, this may be reported as an unauthorised absence and will be reported to my Local Authority’s Out of Hours Emergency Team and updated in my file.

I understand that candles are a fire safety risk and so will not burn Candles in the house.

I will not bring any Pets into the house.

I understand that in order to ensure my safety and the safety of everyone in the home, room checks will be conducted fortnightly (unless agreed otherwise by my social worker) I understand that any items perceived as dangerous or that are prohibited from placement such as alcohol, drugs, weapons, will be removed and disposed of appropriately.
Work to do

I will attend ALL of my college classes and complete all assignments given with support from the staff if I need it. Where it is not possible to do so I will give reasons and advance notice.

I will attend all Key-worker and all mentoring sessions. I understand that ongoing failure to meet with my Mentor or Key Worker may result in me losing my placement.

If I am unable to come to a mentoring or key-working session for any reason I will notify all relevant staff members at least 24 hours before the session. I and your social worker will be informed

I will do my weekly shopping with support from the staff. I understand that this will be used in the initial stages in order to assess my independent skills

I agree to keep my bedroom clean and tidy at all times, with support if needed.

I will keep all communal areas neat and tidy after use.

I agree to engage with services and professionals required according to my support needs.
Infinity agrees to

We will listen to you and be respectful towards you.

We want you to be happy and to make sure all your needs are met, however if you are not happy and feel your needs are not being met, we will always listen, and we will try to fix the issues where possible.

Our staff will always tell the truth

If you have done something wrong, we promise to speak to you in a respectful manner.

Infinity Alliance staff will support you to live independently and will be available to assist where necessary
Breach of Placement
If you break any of the rules above and Infinity Alliance considers it a serious breach then we will take the following steps;

We will set up a meeting between you, your key worker and the Manager of the semi-independent accommodation.

If the situation is not resolved at this initial stage another meeting with yourself, your key worker and social worker will be held

We will use 3 stages of warning

Should there be no remedy for a breach of this agreement, or there is no improvement in behaviours or actions, a notice period will be issued based on the length of your placement

28 day notice may be given of termination of placement if your placement has been more than 6 months

14 day notice may be given of termination of placement if your placement has been more than 3 months
-
5 day notice may be given of termination of placement if your placement has been less than 3 months
-
Should a serious incident occur in which we believe you have put yourself, other residents, or staff at serious risk an immediate termination may be issued and an emergency placement for you may be found through your social worker.

I have read and understood this agreement and confirm that I will respect it.

I understand that serious or on-going breaches of this agreement could result in me losing my placement with Infinity Alliance.
